Appointments
In-person appointments are generally available Tuesday and Thursday from 1:00 p.m. to 4:00 p.m. In addition, I offer Zoom options Monday through Thursday. Sessions are 50 minutes. To make an appointment, please call my office at (602) 319-3335.
Cancellation Policy
Because sessions are part of your therapist’s schedule, I require at least 24 hours notice to cancel your appointment. Please call 24 hours in advance of your scheduled appointment time. If you cancel your appointment less than 24 hours in advance of your scheduled appointment time, you will be charged for the appointment.
Insurance
Elizabeth Marietti is no longer contracted with any insurance providers. Some people may be able to receive insurance reimbursement via “out of network” benefits. Contact your insurance company to identify your out-patient mental health out-of-network benefits. If you request it, I will provide a statement suitable for submission to your insurance company for filing a claim. If your insurance company requests a diagnosis, it will require a code. Your diagnosis may not qualify for insurance coverage benefits.
Note: As of August 31, 2021, Elizabeth no longer accepts insurance. Fees for private pay services are $150 per session.
Payment
Payment is due at the time of service for each appointment in cash, check, Venmo, credit card, or HSA card. A $5 credit card processing fee will be added to your service fee.
Confidentiality
I adhere to state and federal laws along with industry-standard ethical guidelines governing privacy. I can only acknowledge your existence with your expressed, written consent. If anyone requests information regarding you, I will discuss this with you. For me to talk with interested parties, you would need to sign consent forms.
